Social Prescriber

Social Prescribing is a means of enabling GP’s and other healthcare staff at the Surgery to refer patients to a link worker. Our Social Prescriber will conduct face to face conversations during which a patient can learn about opportunities to improve their health and well being. People with social, emotional and practical needs are empowered to find and design their own personal solutions, ie co-produce their social prescription, often using services from the voluntary and community sector.

The potential is to reduce the financial burden on the NHS particularly within primary care. Recognising that  people’s health is affected by a range of social, economic and environmental factors, social prescribing seeks to address people’s needs in a holistic way, and aims to support individuals to take greater control of their own health and lessen their reliance on GP services.

Rachael will be working as part of our team in the Coastal Network.
